Adding an award

The award, the organization that gave it, the year, and the project it was for. Nominations count and are worth listing as nominations rather than being dressed up. Name the organization in the form a stranger would recognize, because the organization is what gives the award its weight, and an award nobody has heard of from a body nobody has heard of is doing very little work on your behalf.
